Testing Algorithm:
If ANCA qualitative screen is positive, then LAB5307 Myeloperoxidase Antibody, Serum, LAB5345 Proteinase 3 Antibody, Serum, and ANCA quantitative testing will be performed at an additional charge.

Autoantibodies to neutrophil cytoplasmic antigens (ANCA) are found in sera of patients with Wegener’s granulomatosis (WG). Patients with vasculitis secondary to other diseases such as systemic lupus erythematosus or rheumatoid arthritis may also produce antibodies which react with neutrophils. These antibodies can be distinguished from the above-mentioned antibodies in patients with WG, the frequency of ANCA approaches 85%. Measurement of ANCA is not useful for discrimination between generalized and limited forms of WG, but changes of titer of ANCA suggest relapse in a patient in clinical remission.
Note: Not all specimens positive for pANCA are positive for myeloperoxidase antibodies.
